Wellness Grant Program. Community Catalyst Grant

The Wellness Grant Program provides opportunities for active engagement and local partnerships that will support and sustain projects that improve the health of Islanders. The program aims to support collaborative approaches, involving many stakeholders and community partners, to address the social determinants of health and prevent chronic diseases.

• To offer opportunities for Island communities to take initiative and lead health and wellness efforts. The idea is for these grants to act as catalysts for communities and organizations to build local capacity and mobilization while positively influencing community well-being. This grant is one small step toward empowering Islanders to identify existing needs and assets in their communities and take action to improve their health and well-being.-family.
